Since November 2019, the British Red Cross has been working in partnership with the Kings College Hospital Homeless Pathway Team to improve the quality of care for homeless patients, reduce potential delayed or premature discharges and avoid unnecessary admissions and A&E attendances.

We've got an opportunity

We are looking for a passionate Team Supporter Worker to join our Kings College Assisted Discharge Service. The Independent Living Service Support Worker post will provide reactive, focused and flexible support to homeless inpatients following a hospital stay to enable early discharge when clinical needs have been met. The post will assist the Independent Living Service Coordinator and the Hospital Homeless Pathway Team by undertaking duties required to meet the service users' needs in order to re-enable service users to remain in their discharge location and prevent hospital re-admission. Referrals may require the support worker to take the patient home from hospital to enable their discharge.

The post-holder will be required to have the ability to assess service users' needs, complete and follow a support plan for the individual. The post-holder will be providing practical and emotional support in the service user's discharge location and signposting to other services where appropriate to enable the individual to live independently in their accommodation.

The Support Worker will act as contingency for the Service Coordinator when required and may need to provide cover for their colleagues at other hospitals served by the Red Cross in the area. Service confidentiality and safeguarding vulnerable people are key responsibilities of the role. The post may require some weekend shifts and some evening shifts when covering for the Service Coordinator (11:00am-19:00pm) but the normal working patterns are Monday to Friday 9am to 5pm.

This post does not include personal care and management of medication.
