This role is to support carers living in Rutland through direct contact with individuals and contribute to the development of services across the county.

We are looking for a dynamic and motivated individual who can??????..

  • Support the independence, physical health and emotional wellbeing of carers and their families
  • Complete holistic assessments to identify individual needs and circumstances
  • Provide personalised information, support and signposting
  • Work independently and as part of a multidisciplinary team with carers; relevant agencies and other professionals
  • Develop and implement approaches to identify 'hidden carers' in the community through innovative engagement
