Health and Wellbeing Facilitator
35 hours per week - Monday to Friday
About the role
As Health and Wellbeing Facilitator you will deliver support services to customers which promote community inclusion and sustainable housing solutions, actively assisting in the marketing of services to secure new customers.
Implementing and managing a simple health and wellbeing assessment tool, you will seek funding for activities and ensure they are managed within budget. Signposting customers to statutory agencies, charities or other organisations for their Health and Wellbeing needs, you will work with partner agencies, including health and care services, staff and customers to improve the health and wellbeing of customers.
Furthermore, you will manage the delivery of customer support plans, ensuring regular reviews are undertaken and considering new and emerging risks or incidents. Keeping up to date records, you will promote community engagement of customers by encouraging their involvement in activities and groups that will enhance their quality of life and increase independence.
Candidates will hold GCSE Maths and English (Level A – C) or equivalent and have proven experience in delivering services that promote independence.
With experience of working in partnership with external agencies and colleagues, you will be a strong negotiator and have excellent working knowledge of Microsoft Word, Excel, Outlook and internet communications. Experience of conducting needs and risk assessment and support and risk management planning Is desirable, as is an understanding of local social care and health structures and/or strategies.
A full and current driving licence and daily use of own car insured for business mileage is essential. Enhanced disclosure and barring service (DBS) check is required in accordance with the safeguarding policies.
