The opportunity has arisen for a Registered nurse Care Coordinator to join a great team in Devon with an amazing team that provide tailor-made care packages to clients in their own home.
Daytime Homecare specialises in supporting adults and children with complex health needs to live in their own home and remain part of their local community.
We currently have several Adult/Children Complex Care packages in the community and are recruiting experienced NCM to work with us. The role is field based Nationwide.
As an Adult/Children Nurse Care Coordinator you would be responsible for providing face to face support to our client families, whilst creating personalised care plans for each client. You would work closely with a core team of carers training and supporting them with clinical skills and activities.
This role is a pivotal role within the community working closely with multi-disciplinary community teams, CHC nurses and CCG commissioners to establish and maintain bespoke packages of care.
Job Description
You will be responsible for implementing care co-ordination and ensuring quality control systems and providing support to all Field Care Supervisors, Care & Support Workers within a defined area.
Responsibility for ensuring all Customer visits are permanently scheduled and allocated to dedicated Care & Support Workers in order to deliver the very highest standards of Customer service.
Principle Responsibilities:
Care planning for service users. (Local authority and complex care).
* Permanent allocation of Care & Support Workers to all Customer visits, ensuring all visits consistently covered.
* Effective communication with Customers, Care & Support Workers and the management team.
* Accepting and scheduling new Customer referrals.
* Intelligent route and program planning for Care & Support Workers
* Strong collaborative working with the Recruitment Officer to ensure any vacancies within the area are highlighted in order to ensure appointments made prior to turnover.
* Maintaining customer and Care & Support Worker office files, ensuring consistency & compliance
* Responding to Customer changes of need
* Ensuring all Customer and Care & Support Worker documentation and electronic records are kept up to date at all times.
* Preparing appropriate documentation for Field Care Supervisors.
* Ensuring all other records are kept up to date.
* Collaboration with Field Care Supervisor with regard to illness and holiday management.
* To ensure compliance with all office systems and procedures.
* Work collaboratively with Managers, Supervisors and Care staff
* Prepare management reports as required
Key Requirements
* Adult/Children Nurse (RNC) with a minimum of 6 months in UK and an active NMC PIN
* Tracheostomy and Ventilator experience essential
* Excellent written & verbal communication skills.
* Ability to work flexibly and respond quickly to changing priorities, while meeting deadlines.
* Supportive management style that can bring out the best in others.
* Confident, passionate and possess a can-do attitude
* Car Owner/Driver (Essential)
