Job Title: Nurse

Location: TA8

Hourly Salary: GBP17.50 Per Hour

Hours of work: 12 hour day shifts/ 12 hour nights

Purpose of Role:

You will be responsible for supporting the Clinical Manager in providing excellent Care Quality in line with CQC and CIW regulations. Working as part of a dedicated and committed team, you will deliver the highest level of care to our residents, delegate tasks effectively to our competent Nursing Assistants and act as the 'Nurse In Charge' when Senior Nursing Staff are absent. You will also liaise with external professionals such as social workers, GP surgeries and hospitals.

Core Responsibilities & Duties:

?? Take responsibility for the well-being of our residents including making sure their physical, emotional and social needs are met.

?? Promote our residents' independence, choice and dignity by delivering the very best standards of care and striving for continuous improvement.

?? Carry out assessments to develop, implement and evaluate individualised care plans for each of our residents.

?? Deliver clinical guidance to the care team, by leading, motivating and mentoring the team to ensure the safe and efficient running of the care home.

?? Take handover at the beginning of shifts and give a detailed handover at the end of shifts.

?? Ensure Senior Carer is informed on gaps in care immediately.

?? Medications to be ordered monthly, interim medications to be ordered when required and emergency medication to be ordered immediately.

?? Administer medication in accordance with appropriate regulations and in line with NMC regulations.

?? Delegate nursing tasks to competent Nurse Assistants.

?? Maintain resident, medication and other records as appropriate and as per the Home policy, ensuring that accidents, incidents and safeguardings are reported, logged and updated.

?? Monitor work areas and practices to ensure they are safe and conform to the relevant standards, policies and legislation.

?? Ensure Health and Safety issues are dealt with promptly and all equipment is used correctly.

?? Report any serious incidents to Senior Management immediately.

?? Support the policies of the Home in Fire Prevention and Security Measures and attend fire training

?? Support the policies and procedures of the Home and attend appropriate courses to maintain individual nursing registration.

?? Ensure adherence to the Codes of Practice laid down by the Nursing and Midwifery Council.

?? Liaise with GPs and other external professionals as required and to be a point of contact for relatives and representatives.

?? Ensure clinic daily checks are carried out.

?? Referrals to be submitted when required.

?? Attend the monthly Clinical Care Meeting.

?? Ensure online training modules are completed on MyAko.

?? Assist the Clinical Manager with staff training and the induction of new colleagues.

?? Carry out any other duties as directed by the Clinical Manager.

Position Specific Standards:

?? Care plans are updated a minimum of once a month and as residents' needs change. Family, next of kin, representatives and professionals are included in the completion and updates of the care plan.

?? Nursing care is delivered as outlined and specified in the care plan.

?? Deliver efficient handover at the start and end of every shift that clearly outlines the changing needs of residents. To ensure any episodes of acute illness is handed over by the RN.

?? Escalate any gaps in care such as fluid intake, pressure relief, safety checks and resident interaction to the Senior Carers on shift and ensure that the Senior Carer takes appropriate actions to address the issues raised.

?? Escalate any performance concerns to the Clinical Manager and Registered Manager.

?? Ensure care staff are supported to understand the responsibilities of their role and carry out the care as instructed in the care plan.

?? Residents' monthly medications to be ordered on time following current regime, medication to be checked in by the Registered Nurse and another member of staff.

?? Residents have all medications that are prescribed at correct specific times following the medication guidance, ensuring correct refusal codes are used, out of stock medications are ordered in a timely manner and medications are destroyed appropriately in line with Home polices and procedures and regulations set by the NMC.

?? Tasks that are delegated to the Nurse Assistant are appropriate and within the remits of their job description ensuring all tasks delegated have complete oversight by the RN. (Nurse Assistant JD and Competency to be read and signed)

?? Have full oversight of PRN medication administration, physiological observation recordings, blood glucose monitoring and any deteriorating residents to ensure that the Nurse Assistant is fully supported, and appropriate action is taken if needed.

?? All falls, skin tears, unexplained bruises, medication errors and all other accidents and incidents are reported immediately using Sharepoint and are escalated via the Safeguarding process and reported to CQC / CIW if required.

?? Clinical Manager are Registered Manager to be informed immediately of any accidents, incidents or safeguardings.

?? All updates regarding accidents and incidents outlined above, are input on to Sharepoint as soon as an update is available.

?? Read and evidence that Homes policies and procedures have been understood.

?? NMC registration is maintained and any updates or restrictions are communicated to the Clinical Manager and Registered Manager immediately.

?? NMC Code of Conduct is adhered to at all times, demonstrating a trustworthy, honest and transparent work ethic at all times.

?? Residents allocated to you as the Named Nurse, are involved in every aspect of care planning and are contacted monthly to review residents' care needs.

?? Daily clinic checks such as fridge temperature, room temperature, oxygen storage and emergency checks are recorded and acted upon where necessary.

?? Referrals such as DOLs, SALT, TVN, dietician and all other relevant referrals are made when required...... click apply for full job details
