Recovery Practitioner

Temporary full time contract.

Monday - Friday 9am - 5pm

General Purpose:

To work as an integral of the Supported Housing Team who provide a comprehensive and high
quality service to Residents. This includes assessing and supporting Residents on a daily basis to achieve their goals, and to monitor their progress through regular key-work and support planning.

Key Responsibilities:

To assist the Referrals coordinator in the referral process - reviewing applications,
accompanying him to interview.
To contribute to the referrals database and help with active waiting lists.
To help with arranging for new Residents to be met at the prison.
To assist with Self Assessments and agree Support Plans with Residents, and to record
outcomes and review progress in achieving their goals.
Key work Residents - work with men to maintain the support needs in their Support Plans.

Person Specification:

Knowledge of people involved with the criminal justice system.
Experience of supporting vulnerable people to achieve independent living.
An understanding of the reasons why people become addicted to psychoactive substances, commit crime and become homeless.
