Community Care Worker - Hospital Team

Seven Social Care are looking for a temporary Community Care Worker to work with the Hospital Discharge Team in Derby. This position is initially for a period of 3-4 months and is paying from GBP11.61 - GBP14.40 per hour (paid to you via an umbrella company).

The role:

To undertake assessments and reviews of service users' and carers' needs that lead to a range of outcomes appropriate to service area.

Key Responsibilities:

Assess, monitor and review the needs of people in whichever client group, using Fair Access to Care Services guidance.

Undertake multi-agency work with internal and external partners to ensure services are delivered appropriately and efficiently.

Arrange services identified in the assessment as eligible needs.

Provide information to service users and their carers.

Record all work undertaken including assessments and support plans ensuring that these meet financial requirements and eligibility criteria.

Work with information technology and associated systems.

Maintain confidentiality and observe data protection guidelines.

Comply with the Council's Customer Care Code.

Participate in the Council's Managing Individual Performance Scheme.

Adhere to the Council's policies relating to Equalities & Environmental Issues.

Applicant Requirements:

• Qualifications: NVQ in Health and Social Care.
• Experience: 2+ years working within a similar role.
• Compliance: Current DBS check.
• Travel: Clean UK driving license with the ability to commute to and from work independently.
• Hours: 37 hours p/w.
