We are seeking an enthusiastic and motivated Senior Social Worker to join our dynamic and supportive social care team based at Salisbury District Hospital.

About the team

The team has had to make significant changes this year in response to the coronavirus pandemic and the resulting government guidance on delivery of discharge services for people in hospital. The focus is on a model of 'Discharge to Assess' where people are transferred from hospital to commissioned nursing home placements for Care Act Assessments to take place. Assessment work still takes place in the hospital by exception and the team continues to provide professional support and advice to health colleagues in the hospital, whether to support with safeguarding adult referrals or to assist decision making about appropriate discharge pathway options or in arranging restarts of care. The team is also beginning to work alongside our Reablement Service colleagues to support the more complex referrals for people being discharged to their own home.

The team consists of a team manager, two senior social workers (this current vacancy would provide the second senior social worker), social workers and customer coordinators as well as administrative support. About the role

We currently have a permanent opportunity for a full time Senior Social Worker within the adult social are team based at Salisbury District Hospital. This is a leadership role within the team and you will be expected to support the Team Manager on a day to day basis, deputising in their absence.

You will be expected to provide supervision to members of the team and be able support them to enable them to achieve safe and timely discharges for customers who are patients in the acute hospital or who are in Discharge to Assess placements. You will be required to manage a small complex caseload when required and have experience in undertaking safeguarding work. The work also involves managing the team's referrals and caseload allocations and working closely with the hospital's Integrated Discharge Service to assist to minimise our customers' hospital length of stay.

As part of the leadership team, you will be involved in supporting the team during this period of change which moves from being focused on delivering a social care service in a hospital setting to investing in outreach work in community settings.

About you

You will have knowledge and experience of applying the Care Act 2014 and the Mental Capacity Act 2005, to be able to support the team to achieve best practice in their work under this legislation. Hospital social work experience is essential and knowledge of delayed transfers of care is an advantage.
