Case Manager

Location: North West

Salary: GBP22,666 - GBP25,617 Dependant on experience

Hours: Full Time, 35h working week

Benefits: Company Pension, Holidays start at 25 days + Bank Holidays and we give up to an extra 6 days a year for staff with longer service.

The job will involve working with people in custody or subject to supervision in the community, to engage them in learning, training and employment opportunities, either through one to one interviews or group work. You'll need to be a good communicator and effective at working with partners to ensure the best results for our participants. Recording of accurate information on participant plans and progress is an important part of the job, so you'll need to be comfortable working on systems and have a strong attention to detail.

Principal Duties and Responsibilities ( not limited too ):

Generate appropriate referrals including from offender management services in custody or community based locations.
To contribute to team targets by successful achievement of individual targets across a number of key performance indicators on a monthly basis.
Undertake an initial assessment of customer needs and develop a programme of support, including a personalised action plan.
Ensure risk assessment information is shared, particularly relating to any restrictions which may impact on employment or training placements.
Maintain contact with customers throughout their journey to sustained employment through motivation, and by providing support and guidance.
Deliver personal development and employability related training to individuals and groups.
To refer an agreed number of customers to Achieve NW Connect's sub-contractors or partners every month.
Corporate Duties and Responsibilities (not limited too) :

Ensure efficiencies are achieved in the delivery of services across the wider Career Connect portfolio
Promote the safeguarding and welfare of children, young people and vulnerable adults in accordance with Career Connect Safeguarding policy and procedures.
Helping to maintain a safe and healthy working environment in accordance with Career Connect Health and Safety policy and procedures.
Respect the confidentiality of all matters in relation to employment and clients. Comply with the requirements of the Data Protection Act 1998 and other legislation with regard to the processing of any personal data.
Promote and maintain Career Connect policies on Equality, Dignity at Work and Code of Conduct.
Candidate Requirements (not limited too):

Understanding of Offender Management Services (National Probation service, Community Rehabilitation Companies and Criminal Justice System).
Knowledge of issues around social exclusion and disadvantage.

Understanding of the principles of equal opportunities.
Experience of working with disadvantaged groups/individuals in the community.
Experience of delivering training or presentations to individuals and groups.
Competency in a range of IT applications including databases, spreadsheets, word processing and knowledge of Outlook e-mail and Calendar. This should include an understanding of the value of online job search including LinkedIn and other social media opportunities.
Ability to achieve monthly performance targets across a number of key performance indicators.
Attention to detail in the accurate recording of data, where clear records of activity can be evidenced and understood.
Ability to manage own workload and to work as part of a larger team.
Excellent communication and presentation skills, both written and verbal.
Evidence of relevant qualifications at level 3 or above
