Marie Curie is leading the way on Dementia care for terminally ill patients in Wales.

Marie Curie is the first organisation in Wales to launch an award-winning Dementia service to support patients and their families. We are offering an exciting opportunity to share your passion, energy and drive for working with people living with Dementia alongside influencing, shaping and improving their care towards the end of life.

You will support the Senior Dementia Nurses to manage a caseload of patients across either Pembrokeshire, Ceredigion or Carmarthenshire. Referrals will be received from a range of care professionals, including GP’s, District Nurses, Social Workers, CPNs and others to ensure the right care package and support is being provided, at the time the patient needs it.

A confident communicator, calm decision maker and member of the wider multi-disciplinary team, you will need some experience and knowledge of working in the community or dementia services. Some palliative or community care experience is desirable but not essential.  You will need good communication skills and the ability to prioritise your work and respond to changing demands.

A full driving licence with access to a car insured for business use is essential, although a pool car scheme is in place. You must be willing to travel across the county to patients’ homes, hospitals and other venues as required.
