A3 Assessor Triage
HOURS: 37.5 hours per week - Covering a 24/7 Rota
BASE: Single Point of Access (Val Waterhouse Centre)

The Single Point of Access Triage team is a team of staff from different professional backgrounds (including Nurses, Social Workers, Occupational Therapists, Mental Health Practitioners, Integrated Advice Officers and A3 Assessors.
As an A3 Assessor working within the Triage team, you will be responding to social care referrals and be part of a multi-disciplinary team. Your primary role will be to determine the most appropriate social or health care pathway for referrals. You will undertake further fact finding activity, including interrogating database systems, liaising with members of the public and other professionals within the Triage team and within external organisations. You will also offer professional advice and guidance around wellbeing and adult social care within North East Lincolnshire.
Although primarily a telephone based role, after induction you may also undertake short term, one off statutory Care Act 2014 adult social care assessments and reviews, on a face to face basis. This will require the completion of person centred support plans.
This role is not a call centre role, it is an opportunity to support people and make a real difference to their lives. Every day is different. We are looking for people who are caring and empathetic and ideally have a background in customer care or health and social care. But, for the right person training will be provided.
Please note that during your induction period, you will be required to work during normal office hours, to provide you with full and detailed training. Upon completion of your induction, you will be required to cover hours between 8:00am and 6.00 pm Monday to Friday. There may be a requirement to undertake some activity out of normal working hours, this activity will attract shift premium payments
