A 3 months + contract opportunity for a Scheme Coordinator - Housing has arisen with the local authorities in Stoke on Trent.
The job purpose is to provide a support and management service to vulnerable older customers living in the community to enable independent living. This is achieved through active involvement with customers and where appropriate other relevant agencies, carers and family members.
Skills and Experience:
Experience of working with vulnerable older people and/or sheltered accommodation.
Show experience of providing services and support to older people.
Show experience of dealing with emergencies.
To carry out an individual needs assessment and to develop a support plan with each customer which takes account of care and/or medical conditions and is particular to his/her own needs. To co-ordinate the delivery of appropriate support and care in accordance with these plans.
Ability to use a range of IT packages including Microsoft Office and internal pre-designed packages.Hours of work are 8.30am till 4.30pm, Monday to Friday, 37 hours per week.
