SHOC is a registered charity working to end homelessness. SHOC creates opportunities for those suffering the effects of homelessness in Slough. We provide support towards recovery and transformation and support people that are rough sleeping. We have helped more than 1000 people make life-changing decisions.

 

The purpose of the Health Coach

 

The purpose of the Health Coach is to work as a key member of the SHOC Team to provide support to our guests through individualised lifestyle changes that meet their unique needs and health goals. This role calls for an individual who is capable of supporting others who may be experiencing personal difficulties to support them with a healthier way of life. A Health Coach needs to be able to assess our guest’s health concerns and then work with them to make behavioural, nutritional and other changes in order to promote better health and wellness.

 

Key responsibilities and accountabilities of the Health Coach

 

Demonstrates innovation and an acceptance of new ideas and concepts.

 

Show the ability to resolve conflicts with clients and overcome their objections about their health and behaviours.

 

Possess the belief and the ability to motivate clients to make vital changes in their lives.

 

Exhibit effective communication skills for working with our guests, their families and their other healthcare providers.

 

Demonstrate a commitment to clients and a willingness to go above and beyond normal responsibilities to provide the best care and support possible.

 

Show self-confidence and the ability to express views that may not be popular or shared by clients.

 

Main Duties

 

1.    Provide support to guests on health-related issues liaising with any external agencies as necessary to ensure the proper care is organized for the individual.

 

2.    To implement Support Groups/Personal programs for clients regarding all health matters (mental health/sexual health etc)

 

3.    Provide a check in service to guests on arrival at the day Centre.  As part of this monitor laundry, shower, staff time and telephone to ensure all clients have fair and smooth access to the service.

 

4.    Kitchen and cleaning duties as part of a team rota.  All cleaning to be carried out to an agreed standard in line with written procedures.

 

5.    Case work and one to one support with guests as agreed in casework meetings. Ensure each client has an action plan which is recorded and monitored on a regular basis.

 

6.    Registration of new guests and entering and updating records on database.             

 

7.    Welcoming guests to the Day Centre and introducing them to the service and explaining procedures and rules.

 

8.    To uphold and enforce all rules of the Day Centre ensuring equality and fairness to all guests.

3.    Represent the organisation, raise its profile and promote its cause.

 

4.    To maintain professional boundaries at all times.

 

5.    Prepare for and participate in coaching and appraisals.

 

6.    To work to safeguarding legislation and policies for vulnerable adults at all times.

 

7.    Keep abreast of developments in services, legislation and practice relevant to the client group.

 

8.    Seek to improve personal performance, contribution, knowledge and skills.

 

9.    Attend and participate in staff meetings and team meetings as required.

 

10. Work within and be familiar with Trinity/SHOC’s policies and procedures.

 

11. Notify your manager of any occurrences which may affect the service or reputation of the organisation.

 

12. Provide written records and reports as required by stakeholders,  the organisation and your line manager;

 

13. Work flexibly so as to maintain the most appropriate level of service provision, respond to organisational change and development;

 

14. Undertake such other duties as reasonably requested by your manager.

 

Person Specifications

 

1.    Ability to meet agreed objectives, targets and outcomes.

 

2.   Proficient use of Microsoft Word and Excel and good IT skills.

 

3.   A high standard of written and verbal communication and the ability to adapt style to suit different audiences.

 

4.   A professional and supportive attitude towards staff, volunteers, clients, visitors to the Day Centre and external agencies.

 

5. Experience of working on own initiative and proactively managing time and workload effectively.

 

Qualifications/Professional Experience Required

 

A relevant therapeutic or health care qualification or experience (at least 2 years practical experience)
