Swanswell support workers are responsible for offering practical support to service users, so that they can change their lives and be happy. We are looking for 9 Support Workers for Swanswell. The role requires excellent communication skills and the ability to connect with people. You’ll work with service users in the community and their own homes, supporting them to develop skills and work towards independent, fulfilling and enjoyable lives. You will deliver high quality, person-centred housing support.

Support workers manage a caseload of people, which may include children, young people and adults, helping them to learn and develop skills that will help them to make sustainable changes to their lifestyle. This may include supporting them to:

  • prevent homelessness by finding and/or maintaining accommodation
  • live within supported accommodation
  • live safely and independently in their own home
  • access primary health care services
  • improve and maintain mental health and wellbeing
  • maximise their income
  • budget money and pay bills

Support workers supporting our service users with HIV or TB will be required to ensure access, maintenance and adherence to medical treatment for people identified as being at elevated risk of not doing so. Our enablement focus aims to help people to self-manage and fully engage with NHS treatment options and other services available to maintain health and wellbeing.

You’ll need to have good writing and computing skills, and be able to work confidently with all Microsoft programs. You’ll also need to be very organised, and have the ability to work to deadlines.
