The Marie Curie Rapid Response Service, in partnership with Betsi Cadwaladr, delivers rapid and flexible out of hours nursing care and support for terminally ill patients and their carer's. We are a community based palliative care service based in the Conwy & Denbighshire area whose aim is to respond quickly to support palliative patients and their carers within the community, usually in the patients' own home. We aim to prevent inappropriate admission to hospital and the choice for the patient of preferred place of care.

The service works in collaboration with the NHS, Out of Hours GP and District Nursing Services, Social Services and other providers of end of life care in the community.

The Marie Curie Registered Nurse will work alongside a Senior Health Care Assistant in the delivery of the service, at the same time as two other R teams, out in the community, covering the county. The Registered nurse reports to and will have the support of the Clinical Nurse Manager.

We are an out of hours service covering nights Monday to Sunday 10pm to 7am. Applicants should preferably have palliative care /end of life care experience.
