An exciting opportunity to work for a local charity as a Referrals and Assessment Coordinator.
Our client is a local charity supporting unpaid carers across Wiltshire who are caring for a relative, friend or neighbour who have an illness or disability.
Position: Referrals and Assessment Coordinator
Location: Wiltshire, flexibly based at Semington Office / Salisbury Office, part-time/ job share considered
Salary: GBP22,911 - GBP25,295 per annum
Hours: 37 hours per week Monday to Friday
Contract: 12 months initially
Benefits: 28 days' holiday in addition to bank holidays, 5% employer contribution to pension, free annual eye tests.

Our client helps carers access services, information, education and training, respite, and breaks from their caring role. They ensure they have a voice that is heard and work with health and social care professionals and employers to raise carer awareness and develop best practice.
About the Role:
The Referrals and Assessment Coordinator will manage referrals in to the organisation, complete registrations, assess, provide information and signposting, triage carers' needs and coordinate support. They will also allocate and prioritise further casework, if required, for carers contacting the organisation. You will be responsible for giving relevant information and advice along with signposting, assessing and following up on initial contacts passed on via team members.
About You:
As Referrals and Assessment Coordinator you will have a good standard of education and knowledge of carrying out assessments over the phone. With a good understanding of the Care Act 2014 and other linked legislation you will have knowledge of multiagency working, including: whole family assessments, liaising with other multi agencies.
You will be able to maintain accurate, timely and detailed records of all referrals and re-referrals, including initial assessments and on-going actions required on Charitylog and/or other reporting systems as required. You will be a good communicator able to liaise regularly with the Community Connector Team Leader to review waiting lists and prioritisation requirements to meet demand. A team player to you will attend regular supervision meetings, to share good practice and ensure your own support needs are met
A qualification in health and social care or other relevant experience would be desirable.
