Education Health Care Commissioning Officer
We seek a dedicated, organised and committed individual to join the Children’s Commissioning and Quality Monitoring Team.
You will join a busy, fast paced team facilitating person-centred planning meetings for children and young people with special educational needs and disabilities (SEND) across the 0-25 year age range, ensuring children, young people and their families are at the centre of assessment and planning processes by facilitating the co-production of education, health and care assessments and plans with and for children/young people with SEND.
You will also work alongside relevant services to ensure effective and timely delivery of the EHC process and EHC plans and work with relevant professionals such as education and health and social care to ensure their knowledge, experience, skills and relevant assessments inform the EHC plans.
You will be qualified to degree level or equivalent in a related field and have experience of working with children, young people and their families with special educational needs and/or disabilities and have experience of working with families in relation to sensitive issues.
A full clean driving licence is required and you must be able to drive have access to your own vehicle for work use.
