Our client is a vibrant charity supporting adults and children in crisis. The Hounslow Enhanced Dementia Care Service is an exciting new venture being delivered in partnership with our client, Hounslow and Richmond Community Healthcare NHS Trust and other voluntary sector partners to deliver care planning to people living with dementia in Hounslow.
This is an amazing post, (internally referred to as Enhanced Dementia Care Planning Lead) working with a supportive and knowledgeable wider team with existing strong links in the borough and exciting project co-working with a range of voluntary and statutory services.

Person specification:
A professional health or social care qualification e.g. Social Worker, Occupational Therapist, Registered Nurse, Physiotherapist (desirable)
In depth understanding of dementia and the effects that it has on individuals and their families
The ability to work in a way that promotes and maintains dignity
Experience of providing services to people with dementia in either a health or social care environment
Ability to co-produce care plans with the person with dementia, their families and immediate carers
Excellent communication skills
Experience of or ability to deliver training to carers or user groups
Strong ability to work in partnership with other agencies
Experience of writing clear reports
Data collection/collation
Data analysis
Knowledge and understanding of safeguarding issues and ability to address them appropriately.
