We want a Practice Nurse

The individual will receive significant support from the Clinical team to develop in this role
Friendly Family oriented Practice seeks an Advanced Nurse Practitioner or a Practice Nurse you will be responsible for the delivery of general practice nursing to the

whole practice population. The focus of the role is both the delivery of evidence-based

practice for patients presenting with acute (for ANP) long term condition, and the provision of preventative healthcare to the practice population.

You will work within the general practice team to meet the needs of patients and support the

delivery of the services. You will be expected to demonstrate clinical and communication

skills to a high standard and a commitment to improving nursing practice through on-going

personal and professional development.

• Chronic disease management
• Infection control
• Immunisations
• Cervical Screening
• Long term disease management
• And other Practice Nurse duties
