SOCIAL PRESCRIBING LINK WORKER

SALARY/GRADE: £21,000

HOURS: 37.5 hrs

PLACE OF WORK:  CAMBORNE, POOL AND REDRUTH

RESPONSIBLE TO: Change manager

CONTRACT DURATION: FIXED TERM UNTIL 31st January 2020 (POSSIBLE EXTENSION subject to funding)

Summary of post

This is an innovative role working collaboratively as part of a local integrated team and collaborating with partners, including other Link Workers, and identified Community Champions within the community. A key part of the success of this role will be in enabling each community to decide how to best use the support available and maintain it using the resources they have.

You will provide a single point of access to the services available through the Cornwall Link and avoid communities being overwhelmed by numerous and conflicting messages as so many different organisations try to achieve similar things.

Key responsibilities

In this role, you will provide support to people to access crisis support, volunteering, recreational activities, training, employment and other community opportunities. This will include using skills such as motivational interviewing, ‘Whole life’ assessments, vocational information, advice and guidance, leisure and recreational signposting, confidence building and assertiveness support. You will develop pop up clinics within surgeries and deliver home visits, where required. The post-holder will increase opportunities to engage with

those hardest to reach and least likely to seek support; understanding the barriers and finding solutions to reduce these. The key responsibilities are as follows:

  • To support people to make the changes they want to see in their lives
  • To provide support and challenge, as necessary
  • To undertake IAG and pre-programme assessments and eligibility checks
  • To undertake assessments, action planning and reviews at CN4C
  • To work with multi project staff at CN4C to enable engagement and progression from the opportunities available in-house and externally
