Care Homes Trusted Assessor, £28-35K – Leeds

The Care Hub has been exclusively selected to work in partnership with the Leeds Care Association, Leeds City Council and Leeds NHS to recruit two Care Home ‘Trusted Assessor’ positions to cover the Leeds area.  Because of the positive impact that Trusted Assessors have made in the Lincolnshire area, Leeds would like to replicate this, and we are now looking to hire two outstanding individuals with immediate effect.  

About you

The role of Care Home ‘Trusted Assessor’ will probably best suit a Registered Manager or Deputy Manager or maybe a Community/District Nurse who’s worked in both care home and NHS environments throughout their career. You’ll probably be someone who’s been looking for a change, but you’ve not been 100% sure what your next role should be. 

These newly created positions have been developed to bridge the gap between the care homes and NHS teams to support older adults who have been assessed and now require care home placement.

Your advice, guidance and support are crucial to your success in the role, therefore you must have experience of working in a care home environment.  You will be required to get out and meet with the care homes and build relationship with the managers and owners to really understand the type of services offered by each provider so that you can match people that require care to available and suitable vacancies. This will usually be to support individuals that are leaving NHS care who now need care home placement.  

This is a unique opportunity and anyone who has worked in care home sector will immediately recognise the benefits of introducing these new roles into the Leeds area.  Because the Trusted Assessor role is newly created post, you will also have a fantastic opportunity to shape to role, and make your mark.

About the role

Finding the right care for loved ones and family members isn’t easy, and helping someone to make the transition from a hospital ward to a care setting comes with its own unique challenges.

You will ultimately report directly to the Chairman of the Leeds Care Association, but on a day-to-day basis, you will develop relationships with wide range of services and individuals, such as Leeds City Council, Adult Social Care, Leeds Community Health.

We need individuals that are pragmatic and continually look to create working partnerships. The clue is in the job title ‘Trusted Assessor’ means exactly this. You need to be credible and your ability to work with care homes to improve access to services, and to ensure effective and quality outcomes for patients is critical.

There will be challenging situations at times, so your ability to positively influence and create win-win outcomes for patients, relatives, carers, and staff is really important, but for the right individual, this is a great opportunity to make a difference.

Salary:  £28-35K depending on experience.
