This is an exciting opportunity to work as a Locality Worker, on this project for people living with Dementia or Memory Loss. This is a 37.5 hours per week vacancy.

Fixed Term until Feb 2020
Salary: GBP22,054 per annum
Responsible to: Team Leader
Full time (Monday - Friday)
Base: Hertsmere - Occasional travel to other localities

Purpose of Post

This role is part of a new countywide community based dementia project which will support older people, people with young onset dementia and people with learning disabilities with mild to moderate dementia and cognitive impairment with accessing information and advice on rights and entitlements as well as providing community countywide activities to improve mental and physical wellbeing

The role of the Locality Worker is to deliver services including healthy eating sessions, physical activity sessions, digital inclusion sessions and socialisation. Ensuring access for older people, people with young onset dementia and people with learning disabilities who have dementia and mild cognitive impairment including those living in rural areas of the county. Services will be provided at the older person's home or in a group setting using a variety of different community venues

The primary aim of this project is to improve mental, physical well-being and socialisation. This will be achieved through the dissemination of healthy living and healthy eating information and advice and encouragement to participate in activities which promote physical and mental wellbeing and prevent social isolation. It will also be achieved through socialisation and digital inclusion.

You will:

* Deliver training and development for professionals and organisations in the awareness of the issues faced by older people, people with young onset dementia and people with learning disabilities who have mild to moderate dementia and cognitive impairment as well as the needs of carers and their families

* Support the Team Leader and be involved in the set-up and delivery this new service working within the community and in partnership with other partner organisations as well as statutory services.

* Be an information and advice point for older people who have mild to moderate cognitive impairment living in Hertfordshire working in partnership with Herts Help
