We are looking to appoint full time passionate and motivated Care and Support Assistants to work as part of a supportive team to deliver Rapid Response services to people living in the community.
Our team enables people to be discharged safely from hospital back to their own home and provides care and support services to people to avoid them being admitted into hospital, no two days are the same!
