To be responsible to the registered manager for planning, organising and delivering a varied activity programmes for residents in accordance with their assessed needs and wishes as recorded in their plans of care. This should include individualised and group activities and outings that are person centred and appropriate to resident’s requests, needs and abilities, transferring what residents enjoyed during their lives into the care home setting.
To offer activates for residents that reflect their individual and cultural interests and developed in accordance with the homes equality and diversity policies and philosophy of privacy, dignity, independence, choice, rights and fulfilment for all residents, therefore treating everyone with respect.
To develop and maintain life histories and activity care plans celebrating resident’s unique history and experiences, in collaboration with the nursing and care team.
Principle Responsibilities:
1. Help to assess and collate resident’s interests and preferences in association with the Residents Liaison Manager, care staff particularly key workers and family members.
2. To liaise with all new residents moving in and their families aiding the transition from home to care home.
3. To develop detailed Life History booklets and memory frames for each resident in partnership with their families by way of celebrating resident’s lives’ and increasing meaningful engagement of care staff with their residents.
4. To deliver a wide range of person centred daily/weekly/monthly individual and group activates, being flexible with working hours with the ability to accompany residents on outings and attend/manage weekend/evening internal events where appropriate.
5. To actively promote a culture where the activity programme is accepted and recognised as essential to wellbeing and integral to daily care, and is the responsibility of all staff to engage in supporting residents.
6. To encourage and motivate residents to participate in physical and mentally stimulating exercises that adds quality to their lives.
7. To provide individual care plans for all residents relating to their activities, setting outcomes to be achieved, that are reviewed with records of what they have been involved in and achieved.
8. To support residents to have a varied social life with good interaction including community involvement and group activities.
9. Investigate, develop and extend the range of activities and interests that the home can offer, engaging residents in meaningful activates including Dementia stage relevant activities.
10. Seek out and identify appropriate activity visitors and schedule special events in consultation with the registered manager and proprietor.
11. Follow NAPA guidelines and utilising NAPA resources.
12. To undertake risk assessments of suitable venues for residents to attend in the community providing an opportunity for a change of scenery and achieving n enjoyable outing.
13. To provide information for families, friends and residents about the activates being offered, displaying, distributing and updating activity programmes.
14. Support the homes residents committee meetings.
15. Be responsible for the appropriate use of the homes activities budget, working as a team with the staff of the home and other activity co-ordinators to ensure resources are pooled and economies of scale are benefited from.
16. To follow the Dignity Challenge at all times.
17. To undertake such other duties as may be required and which are consistent with the nature of this post.
