We are working with our Newport town centre client to recruit a part time Housing Support Worker
You will join an existing team of people and work around 30 hours each week.
The purpose of this role is to provide a generic housing-related floating support service in Newport for People aged 55+. Support will be person centered, focusing on realistic agreed goals and appropriate outcomes.
KEY RESPONSIBILITIES
* To be willing to work across the services operational area
* To maintain confidentiality and understand issues of General Data Protection procedures
* To perform routine administrative tasks as directed by the Team Leader
* To guide and mentor Life Skill Volunteers in order for them to provide holistic and targeted support to service users within time sensitive parameters
* To help vulnerable people develop, or maintain, the skills and confidence necessary to live independently
* To be mindful of complementary services available in Newport
* To signpost to alternative services if housing-related support needs are met or not identified
* Provide housing- related support to service users in a variety of ways including financial maximisation, practical skills, maintaining payments and promoting economic progress, improving resilience, accessing appropriate services and accommodation
* To have a clear understanding of housing needs of people aged 55+ and in particular those adults at risk or in crisis situations
* To assist service users with support in their home, office or on the telephone as required
* Completing Schedule of information and Gwent Needs Mapping Exercise (GNME) form as required by Supporting People Team
* To keep detailed records in order that accurate statistics can be compiled
* To log clear and concise information on shared IT Systems ensuring all electronic case notes are completed accurately and to a quality assurance level standard
* Complete comprehensive, person centered support plans with service users focusing on realistic agreed goals which enable outcomes to be monitored
* To assist service users with support to address housing related issues
