I am currently recruiting for a Housing Related Support Worker based in Leicester. This is a full time role working Monday to Friday 08:30am to 5:00pm. You must be flexible as city wide cover at other office bases maybe needed. You must have an Enhanced DBS.
The key responsibilities are :
•Identifies the needs and abilities of service users to obtain and maintain independent living, visiting, interviewing and undertakes assessments and referrals as necessary.
•Provides planned housing related support service to those moving into or sustaining independent accommodation.
•Assists in establishing action plans to help services users maintain accommodation or achieve a planned transition into suitable accommodation.
•Works with statutory and voluntary agencies relevant to service users to develop a resettlement and support package, involving multi-agency solutions to resolve complex need including intervention & reduction programmes.
•Maintains a caseload, providing reports and monitoring statistics, to ensure effective performance.
•Takes a proactive role in identifying and undertaking case, risk and risk reduction assessments.
