We are looking to recruit a warm and compassionate Care Co-ordinator working in the expanding Community Care team within the Leeds Super Hub.
At Reed Community Care we aim to offer genuinely effective, professional and personalised support services. This is tailored to individuals to promote their independence to help those who wish to stay in their own homes and communities.
We co-ordinate independence and participation in community activities, enabling choices, encouraging decision making, respecting confidentiality, and maintaining dignity and self-esteem.
We provide personal, practical, social and emotional care and support services to adults, children, families and young people with various care needs including learning and physical disabilities, mental health problems, sensory loss and complex health and care needs.
It is the Co-Ordinators role to effectively match those who use our services to staff who are able to supply the right kind of support. This takes dedication, hard work and organisational skills.
