Position: Dementia Navigator
Location: City of London and London Borough of Hackney
Contract type: Part Time, Permanent
Hours: 17.5 per week
Salary: GBP12,197 actual per annum

About the role:

You will work independently in the community and as part of a very Dementia Navigator team. After a short induction period your role will include meeting clients and carrying out person-centred assessments.
You should have experience undertaking assessments with clients in their own homes in order to help them identify their own person-centred needs and agree a personal outcome plan.
You will be signposting people towards services that can help meet their needs - and where required you may accompany them on their initial visits to services you have signposted them towards.
You will be providing information, advice, signposting and support - helping people with mild to moderate dementia to navigate their journey along 'the dementia pathway', undertaking regular reviews with them as required.
You will be working as part of an experienced multi-disciplinary team - supporting people with dementia and their carers to access local services and groups.
Primarily based in the East London Foundation Trust's 'Community Mental Health Care of Older People' office in Hackney Wick, the successful applicant will work directly with people who are living with dementia in the London Borough of Hackney and The City of London. The role will necessitate travelling to undertake home visits and other activities.
About you:

We are looking for someone with experience of working with people affected by dementia in a paid or unpaid capacity. You will need to possess (as a minimum) an NVQ level 3 (or equivalent) in a Health and Social Care related field and you will have demonstrable experience.
We are particularly looking for someone with great interpersonal skills who has considerable experience of working with people affected by dementia in their home environments. You will be able to help them identify their individual needs and offer support to them and their families. Therefore, you should have experience of working with a wide range of statutory and voluntary agencies - especially GPs.
