Age UK Oxfordshire have an exciting opportunity for a Community Information and Activity Worker to join their growing team, based both in John Radcliffe Hospital and out in the local community.

Job title: Community Information and Activity Worker

Salary: GBP11.01 per hour

Location: Home, Office, Community and Acute Health Care setting (John Radcliffe Hospital)
Job Type: Fixed term contract, March 2019. Various hours, including weekends and shift pattern

Shifts Available in the hospital: 08:00-14:00 or 14:00-20:00 Monday to Friday / 10:00-15:00 Saturday and Sunday

Normal working hours during the week out in the community

Community Information and Activity Worker - Job Purpose:

Working within local communities and health care settings you will be joining our successful Community Information Network Team as we work to further improve access across Oxfordshire to information, support, activities and opportunities for all those with a health, care or support need.

You will be working with individuals, families and groups in community and hospital settings as well as in their own homes. We work with local communities to further build upon and support what is being offered by them as well as working with individuals and their families to connect them with the support and opportunities that exist.

Community Information and Activity Worker - Main Duties:

- Ensure that local resources within communities are mapped and work to further develop and sustain the network of grassroots partnerships with agencies, individuals and groups working in the locality
- Support individuals and their families by helping them to identify needs and desired outcomes. Providing face to face information and signposting in both community and hospital settings and people's own homes, supporting them to create an individual plan as appropriate; Keep information and advice knowledge up to date by attending regular training sessions and team meetings
- By being responsive and creative, develop, promote and deliver an increasing menu of activities and opportunities for older people within their community that are sustainable. Help improve access to available services, supporting people to both know about and link into the support that exists through a range of community based network marketing events, information fairs, drop-in's and talks
- Work with, further develop and sustain the community link/volunteer network, building and supporting a team of volunteers to help run activities and support individuals to access opportunities in the locality
- Ensure that systems are in place for receiving and recording regular feedback from people using the service whilst maintaining accurate and up to date records re work done. Contributing towards the effective monitoring and review of the contract(s)

Community Information and Activity Worker - Essential Experience:

- Working with older people and an understanding approach to their needs
- Experience of working collaboratively with other statutory and voluntary agencies

- Experience of diagnosing individual issues and providing relevant information and support to vulnerable people to act on the information
- 5+ O levels/GCSE/CSE or equivalent; grades A-C including Maths & English
- Very good IT skills including Microsoft: Excel & Word; email, internet etc and the ability to keep accurate, up to date records using client management system
