Keystream are recruiting for a Serious Incidents Investigation Support Officer on behalf of an NHS organisation in Central London. The successful candidate will provide support to the Serious Incidents Lead in their investigations throughout the organisation. The role will involve requesting accounts from relevant staff/ patients/ families and ensuring the ongoing investigations adhere with established guidelines and timelines. The purpose of the role is to Support the Lead Investigator to develop the investigations key lines of enquiry.

The Role

* Ensuring that Duty of Candour is undertaken and that the relevant parties are encouraged to contribute towards the investigation.

* Requesting account of events from relevant parties involved in a serious incident.

* Writing a chronology and summary of care relating to the incident

* Supporting the Serious Incidents Lead Investigator to develop the investigations key lines of enquiry.

* Creating necessary documentation for the investigation team panel meeting.

* Arranging a panel meeting with senior leadership teams to review the findings of the investigation.

* Reviewing and ensuring that investigations have been appropriately carried out in accordance with agreed guidelines and timeframes and to determine if the investigation is fair and/or in need of further information or interviews and providing feedback on key areas for improvement, challenge or further actions.

* Validating root cause analysis and resultant action plans;

* Writing the final investigation report
