This is an exciting pioneering role for Carers seeking opportunities for professional development in Health & Social care. You will work closely with a supportive multi- disciplinary team to ensure the best possible outcomes for the patients in our care.

This role involves providing domiciliary care for a wide variety of patients, offering a short term service whilst a longer term package of care is being secured. Patients will be supported in their home environment to prevent avoidable hospital admissions, facilitate earlier hospital discharges as well as in-reaching to the wards.
