£17,460 to £18,702 p.a., pro rata Variety of hours available

This is an exciting pioneering role for Carers seeking opportunities for professional development in Health & Social care. We want you to be part of a new and innovative team that helps shape the future of our services for the local community; making it a sustainable and forward thinking service to support people to stay well in their own homes. You will work closely with a supportive multi- disciplinary team to ensure the best possible outcomes for the patients in our care.

This role involves providing domiciliary care for a wide variety of patients, offering a short term service whilst a longer term package of care is being secured. Patients will be supported in their home environment to prevent avoidable hospital admissions, facilitate earlier hospital discharges as well as in-reaching to the wards.
