Support Worker Grade 2 - Waking Nights

Salary: GBP15,309.61 - GBP17,244.70 per annum, pro rata, plus a Geographical Allowance of GBP1,483 per annum, pro rata
Hours: 20 hours per week
Location: Bexley

We are an accommodation and support service in Bexley working with people with; complex learning, mental health, sensory impairments, mobility and communication needs.
The team comprises of a Service Manager, a Deputy Service Manager, Support Workers?? Grade 2 and Support Workers?? Grade 1.
As a Grade 2 Support Worker, you will;
???Hold a small caseload and supervise staff
???Coordinate and develop Health Action Plans and Person Centred Plans.
???Liaise with health care professionals and other professionals to promote healthy lifestyles; to maximize quality of life for our customers.
???Ensure you demonstrate good practice to promote security of the building, staff and the safety of the customers living in the service.
???Give practical support to individuals in line with customers care plan and Advances ??policy and procedures.
???Help develop local procedural arrangements within the service, contributing to the wider services aims and objectives.

??Flexible to work on a rolling rota; including weekends and evening shifts

??Able to prioritize your workload
