Queenscourt Hospice is a local charity providing support for people of Southport, Formby and West Lancashire.

Transform Facilitator

£26,829 - £35,931 pro rata

Full or Part Time considered; 6-12 month fixed term contract (contract extension dependant on funding & fit)

An exciting opportunity has arisen to join the TRANSFORM Team to make a difference to the care of those in the last months of life in hospital, care homes and at home by educating, supporting and empowering clinical staff, families and patients.

You will be a registered nurse with previous senior level experience of end-of-life care. You will be expected to work you hours over seven days a week in all care settings.
