Social Work Practitioners (Level 1 / Level 2)

BASE:                    Agile Worker

HOURS:                 37.5 hours per week - worked over a 7 day rota

SALARY:                £27,362- £28,463 (Level 1)

                             £29,334 - £35,225 (Level 2)

Focus is a Community Interest Company that provides statutory services to the North East Lincolnshire Authority.  We are an innovative, dynamic and forward thinking social work organisation.  It has led on introducing an integrated health and social care data base; creating a multidisciplinary hospital team and integrated front line services.  We also work with people who have complex and long term team needs.

The Post

This is an exciting opportunity for social workers to join us within our expanding Hospital Discharge Team.    

You will work with cases on a short term basis to enable appropriate and safe discharges from hospital.  The nature of the work is challenging and fast paced therefore you will need to be professional and resilient to undertake this Social Work Practitioner role.

We are looking for people who can build positive relationships at a number of levels and bring enthusiasm creativity and passion to the service.

If you are motivated, reliable and able to work autonomously to find creative solutions for the people you work with, then you are the social worker who we want to work for our organisation.

These roles will form part of a rota to cover the Hospital Discharge Team service 7 days per week.

You must be registered as a Social Worker with the HCPC or a newly qualified social worker awaiting HCPC registration.
