Transition and Diversion Worker
(Birmingham Changing Futures Together)
£20,253 to £22,212 per annum
15 month fixed term contract
35 hours per week
The successful candidate will work alongside the BSMHFT Liaison and Diversion Service from point of arrest through and out of the criminal justice system to assist individuals with multiple and complex needs to integrate back into their community and help them access local services.
The Transition and Diversion worker will support adults to access various services available via the No Wrong Door and help them to gain confidence in doing so. They will develop effective links and working relationships with external services and agencies and identify key contacts. This role focuses on 'refer and accompany' principle as opposed to 'refer and signpost'
