Fixed Term Part Time Support Coordinator

30 hours per week

£19,446 - £21,693 p.a.

Location: Prospect House, 1 Padge Road, Beeston, Nottingham NG2 2JR.

An opportunity has arisen for a 30 hour Support Co-ordinator based at Prospect House, the Registered Office of the Short-term Assessment & Re-ablement Service covering Broxtowe, Gedling and Rushcliffe. 

The post holder will be responsible, as one of three people, for the efficient and effective rostering of a workforce of 60+ people and a service user group of approximately 80 individuals across all three localities.

You will be expected to manage capacity so that the service is able to accept all new referrals into and out of the service in a timely manner, with a focus on hospital discharges and preventing discharge delays. 

You will need to have knowledge of domiciliary care and re-ablement, rostering systems and providing Management Information Reports.
 

The post holder will need to have excellent communication and interpersonal skills to enable them to work effectively with a variety of professionals and departments to ensure the best outcomes for the service user and timely progress through the re-ablement period.

You will also need to be able to work in a busy environment.
