As an experienced Support Worker, you will be qualified to NVQ Level 2 or equivalent in Care or you will have previous relevant experience. You will have experience of working with people with mental health conditions and learning disabilities in their home and out in the community. This might include motivating someone to eat healthily or visit the gym or providing support to a person with emotional problems.
You will play a key role in developing and delivering person centred support plans in collaboration with your customers, their family members, Advance colleagues and other professionals, enabling them to continue living independently within their community.
Our outreach service largely delivers daytime support on weekdays, though some evening, weekend and bank holiday work may be required at times.
We would love to hear from you if you are passionate about improving the lives of local, vulnerable people. If you think a Support Worker role working for Advance sounds right for you, you will need to be:

• Good at working on your own and within a team
