This is your chance to be part of something brilliant at Home Group, where clinical interventions work hand in hand with daily support. You’ll be joining us on our exciting journey to become market leaders in New Models of Care by 2021. This means we’re not providing your traditional supported housing services but rather trailblazing the way in delivering brilliant long term outcomes for vulnerable people through integrated health, housing and care.
You’ll be joining our brand new Beech Range registered care home in Manchester, which provides step down support to adults leaving hospital who have had an acute mental health diagnosis. We are super proud that we provide a stepping stone for people who have been in a clinical environment (but no longer in need of a hospital bed) to semi-independent shared living, to finally living independently in the community. You’ll be working alongside a Registered Mental Health Nurse on a daily basis who will advise you on support practice and in preventing future admission for customers who are in crisis. How amazing is that!
There’s lots for you to do, from leading on caseloads, risk assessments, support planning, goal setting as well as regular reviews. You’ll also be partnering with local agencies. In this service there is no requirement to allocate work to other support workers as you will manage your own caseload. We guarantee every day will be different which will inspire and challenge you. You will constantly learn new things and be stretched professionally, emotionally and practically; your learning will be immense.
And you won’t be on your own, we are recruiting a big team to deliver excellent person centred support, so you’re going to be part of our “Walton” size Home Group Family.

A bit about you
You’ll be super energised, with a positive can do attitude and buzzing to help us build this flag ship service. Your empathy and creative spark will really shine through as will your ability to empower customers to reach their full potential. You’ll bring previous experience of working with vulnerable people in a support or care environment which of course will include case management, safeguarding, risk assessments and working with external agencies. If you hold a Level 2 or 3 in Health and Social Care this would be ideal, but if not we’ll pay and support you to achieve this.
This is an excellent opportunity if you’re looking for the next challenge and to work hand in hand with medical professionals to improve support practices. So you’ll be trained within an inch of your life (so don’t worry about that) to gain a true understanding of our systems and to capture all the excellent outcomes we are going to achieve. We love a bit of technology here so you will be working with a top of the range IT kit which could include a smart phone and surface pro tablet, so you can work on the go. No being chained to your desk with us lot! There may be some travelling but don’t worry it’s all covered in your working hours and you can claim your mileage too.
We will be providing 24 hr support, which will be on a rolling rota (yet to be decided) but will cover evenings, weekends and bank holidays. So we need you to be flexible.
If you are successful, we need you to have an Enhanced Disclosure Check through the Disclosure and Barring Service which we’ll pay for.
