The Role:
The primary purpose of this post is to deliver a high quality person-centred assertive outreach service working as part of a team, making contact with new, repeat and entrenched Rough sleepers in Basingstoke and Deane, implementing a locally agreed approach to No Second Night Out
Principle Areas of Accountability:
- To undertake assertive outreach and hot spot visits across the locality area including during unsocial hours, maintaining a regular street presence
- Identify, engage and assertively work with people sleeping rough within the locality area
- Carry out needs assessments with identified rough sleepers to secure access to services which are suitable for their immediate needs, and make appropriate referrals in partnership with them to establish a sustainable life away from the streets as quickly as possible
- To work within No Second Night Out guidelines when working with those new to the streets
- Establish supportive, trusting and professional relations ships with service users using an Appreciative Inquiry approach
- Develop and implement person-centred packages of support with people sleeping rough
- To work in close partnership with the local authority, police, emergency accommodation providers and wider partner agencies to maximise positive outcomes for service users
- To work closely with the MEAM Co-ordinator to ensure that rough sleepers with multiple needs are supported through the MEAM approach
- To track the progress of the referrals made in respect of identified rough sleepers
- To build relationships with people who are rough sleeping, using inter-agency outreach sessions and regular visits to agencies/venues visited by people who are rough sleeping
- To give clients advice, information and provide an enabling service so that they can access appropriate services
- To carry out risk assessments and complete Housing Benefit forms for rough sleepers moving into designated accommodation services
- To recognise indications of substance misuse, mental health needs, domestic abuse, personality disorder behavioural issues and make appropriate referrals to relevant agencies
- To promote and facilitate effective multi-agency and joint working to address begging, street drinking and other street-based activity
Knowledge, Skills and Qualifications required:
Essential:
- Plan and organise own work
- Work on own initiative under pressure
- Liaise and negotiate with front line staff
- Work flexibly towards creative solutions in a person-centred way with individuals who may have seemingly intractable problems
- Build effective relationships with clients and services in order to develop pathways to enable clients to make positive changes
- Set boundaries, challenge appropriately and manage conflict constructively
- Keep accurate records and statistics
- Excellent range of interpersonal skills
- Good communicator - oral and written
- Problem solving & analysis
- Contribute to reports and assist in the presentation of findings
- Administrative/computer skills
The following experience is also deemed essential:
- Outreach or community based work
- Working with people with complex needs who may be difficult to engage
- Working with dual diagnosis clients
- Assessment of needs and case management
- Working within a multi-agency setting.
Desirable:
- Health & Social Care Diploma/RMN/ CQSW/CSS/DipSW or OT
- Working with people who are or have been street homeless
- Resettlement and support packages
- Involvement in research/services development
- Understand and be able to carry out client based Risk Assessments
- Understanding of the importance of health and safety in respect of Lone Working
