You’ll work alongside fellow dedicated and highly skilled social care professionals to assess the needs of local residents and their carers by undertaking appropriate assessments which will lead to care and support plans. This is an opportunity to put your HCPC qualification to great use. You will have strong communication and organisation skills, and a flexible approach will ensure you deliver positive experiences to patients at every stage of their healthcare journey.
