As a Reablement Worker you will work with customers intensively for up to six weeks to help them achieve their goals, including providing support with personal care in their own homes. You will be given the freedom and time to work with customers, family members and other Health & Social Care professionals to develop new ways to encourage people to live as independently as possible. You will work in the Clevedon and Portishead areas.
The Job:
- Creating reablement care plans that help people achieve their goals, providing and suggesting extra access to support as necessary.
- Working mornings only on a 4 on, 4 off basis.
- Keeping your team and health professionals updated regularly.
The Person:
- A flexible and reliable individual.
- Car drivers essential.
- Experience in Health & Social Care preferred.
