Transitions Support Officer (Permanent)

Grade G5 Point 12-15 – (£19,430-£21,962) pro rata per annum

32.5 hours per week, 39 weeks per year

Do you have a passion for supporting transition from specialist schools at 16 and 19 Years of age, working collaboratively with families, young people, education and non-education service providers? Do want to ensure our school leavers continue to reach their potential and realise their aims and aspirations?

We have an exciting opportunity to join this innovative and dynamic Federation. We are looking for an enthusiastic, reflective and ambitious practitioner to join our developmental team, with a commitment to supporting the development of young people with learning difficulties within this Federation. The applicant should have energy for supporting positive transitions for young people into post 16 & 19 provision.
