About the role…

Working as a member of a multi-disciplinary team, you will be responsible for delivering effective and high-quality nursing care to patients in a home environment to facilitate early supported transfer from hospital or to prevent unnecessary admission.

Working in line with patients’ needs, key performance indicators, outcome measures, goals and targets, you will safely administer a range of therapeutic interventions whilst maintaining accurate contemporaneous clinical records, ensuring they are logged on relevant clinical systems.

About you…

So if you are RGN part 1 NMC registered and have experience in an acute care setting to include some time taking independent clinical decisions, then they would love to hear from you. You’ll need to have a strong customer and patient focus and display a positive work attitude and personal resilience, including effectively controlling emotions, being self-motivated, handling set-backs and remaining positive.

You’ll need to be able to make good use of available time and be a strong team player who can build effective relationships with others, both within and outside the business. With a strong understanding of therapies and drugs relevant to service offering, including NHS knowledge relating to the management of patients and treated conditions, you will have experience of undertaking holistic clinical assessments and have experience in the care of IV devices and administration of IV therapies.

Due to the nature of the role, a driving licence is essential.
