JOB PURPOSE:
To assist the case manager to recruit and train a support team. Co-ordinating, supervising and working as part of a team supporting a young man Cerebral Palsy living in his own home with some physical, cognitive and emotional difficulties.
The Team Leader is required to provide supervision and co-ordination, being responsible for rotas and liaison with others involved.
The Team Leader will work as part of the team in the provision of hands on care as and when required.
The Team Leader will act with the Case Manager and Assistant Case Manager in leading the team of carers in order to:

  • Support the client and to ensure his general wellbeing.
  • Ensure consistency and high standards of support. In particular, to ensure that carers understand their duties and the responsibilities of their jobs.
  • Provide Supervision and support and to identify any training needs for the members of the care team.
  • Ensure the client's safety and physical wellbeing at all times.
  • Facilitate the client's independence by adopting the appropriate approach as advised by the case manager and therapists.

The Team Leader will provide feedback to the Case Manager on all aspects of the client's well-being and his care regime. In conjunction with the Case Manager they will complete a review of risk assessments and service user plans annually; organise team and administer team meetings; organise events such as holidays.
The team leader will plan rotas, accommodating annual leave/sickness cover by arranging bank support workers; maintain appropriate care records and staff records; collate and submit monthly timesheets for payment.
The team leader will be aware of and assist in, the procedures for:

  • Equipment maintenance (assistive equipment and home adaptations).
  • Managing petty cash and overseeing financial matters.
  • Maintaining records of financial transactions.
  • Assisting with the induction of new staff by ensuring they are aware of day to day routines and procedures.

The team leader will liaise with any specialist therapists providing a service to the client, provide feedback to the therapist and ensure that all recommended rehabilitation is carried out for the client's maximum benefit.
The team leader will act as the named individual for contact in the absence of the Case Manager.
Main Duties

  • To ensure consistency of approach and standards within the team.
  • To liaise closely with the case manager.
  • To liaise with and support the family as required.
  • To ensure the client has the opportunity is given to participate in activities and that his week is appropriately structured.
  • To be aware of any significant changes in the client's health, functioning or behaviour, and inform the GP, family and Case Manager as appropriate.
  • To encourage the client's independence as far as possible.
  • To be aware of the client's vulnerability and take precautions to protect it.
  • To be aware of the client's physical and cognitive problems.
  • To respect confidentiality at all times.
  • To be aware of the client's limitations and strengths.
  • To ensure training opportunities are taken.
  • To undertake training in the client's care regime within the home and other locations visited as appropriate.
  • To undertake training in all relevant skills to undertake the role, as identified by case manager in liaison with other professionals involved in delivery of rehabilitation for client.
  • To adhere to guidelines when taking the client out in his vehicle.
  • To deliver therapeutic programmes as directed by the relevant professionals.
  • To undertake relevant risk analyses at least once a year.
  • To undertake other duties as may from time to time be assigned to you by the Case Manager.

General Duties

  • To be aware of the problems that the client may have due to his physical, cognitive and emotional difficulties.
  • Attend relevant training days and sessions when the opportunity arises.
  • Attend staff and team meetings as requested.
  • To carry out domestic chores as appropriate.
  • Keep your own record of hours worked on the time sheet.
  • Provide feedback to the case manager and family on progress/problems.
  • To be aware of the house rules, policies and procedures.
  • To organise and manage own time according to delegated workload.
  • To complete daily record sheets.

PERSON SPECIFICATIONS
QUALIFICATIONS/TRAINING/EXPERIENCE
ESSENTIAL

  • Experience as a support worker
  • Experience of liaising with family members, professionals and organisations

DESIRABLE

  • NVQ Level 3 (or equivalent) in Health & Social Care
  • Previous experience in a similar role
  • Experience of managing people and managing staff rotas
  • Recent training in first aid and health & safety

KNOWLEDGE/SKILLS/ABILITIES
ESSENTIAL

  • Good understanding of the personal needs of a client with physical, cognitive and emotional difficulties.
  • Ability to follow instructions / guidance.

OTHER REQUIREMENTS
ESSENTIAL

  • Full driving license and use of a car

Full training, supervision and support will be provided as required.
This role is subject to an Enhanced level DBS check.
