30 Hours Per Week
£22,300 (Pro Rata) per annum
Fixed term 18th month contract.

The Wolseley Trust is pleased to announce an exciting new 'Healthy Futures' social prescribing service delivered in partnership with Plymouth City Council, Primary Care and VCSE organisations across Plymouth. The Trust is seeking to recruit a new team of three Link Workers to help deliver this pioneering programme.
'Healthy Futures' enables the utilisation of community resources to meet needs unmet by traditional health service delivery models by developing relational networks between local services which enable health and wellbeing.
Working across both GP and community settings, you will use the social prescribing model to help people find appropriate community and non-clinical solutions to their needs; to reduce their social isolation and improve their health and wellbeing.
You will have excellent holistic assessment and consultation skills and will ideally have undertaken formal training in relation to health; social care, counselling or therapeutic approaches, or have other relevant professional or academic qualifications or experience.
