The impact of delayed transfers of care on patients is a national issue and one that we have taken very seriously here in Sheffield. As a result, the Chief Executives across Sheffield Teaching Hospitals, Sheffield City Council and Sheffield CCG established the DTOC Programme in Spring 2017. As a result, relationships and collaboration between partner organisations in Sheffield have significantly improved leading to a dramatic reduction in the number of patients waiting. However, further transformational work is required to maintain this position throughout winter and ensure new ways of working are embedded and resilient. As part of this programme of work the Sheffield Accountable Care Partnership has identified the need for this key coordinating role. The post holder will have two key tasks: 1. To co-ordinate the Task Team (a team proactively managing DTOC on a daily basis). 2. To tactically develop further opportunities to reduce reportable and non-reportable delays, and therefore extend the scope of the Task Team over time to highlight and work on issues and help solutions become mainstreamed within day-to-day multi-disciplinary working. Hosted by Sheffield Teaching Hospitals this is an exciting joint role working across the key services provided between Sheffield Teaching Hospitals, Sheffield City Council (Adult Social Care) and Sheffield CCG (Continuing Health Care). Importantly this role will need to represent the facts in relation to managing flow and discharges in Sheffield and where multiple organisations are involved provide an unbiased view on what is happening in order to unlock delays and support patients to be discharged. In supporting the triumvirate of system directors leading the DTOC Programme of work the post holder will systematically collate, review and expedite delay issues whilst keeping track on the transformational workstream programme delivery which supports and embeds operational and cultural changes across STH wards, Adult Social Care and Continuing Health Care. As a result, we are looking for a Registered Nurse with extensive experience in a senior management and/or clinical role across NHS or local authority sectors who is educated to Master's Degree Level or equivalent and who has had experience of leading and delivering complex programmes of work across different teams and services.
