The Team consists of an Operational Team Manager, 2 Senior Social Workers and 3 Social Workers working closely with our Behaviour Advisory Team. We are a county wide team based in County Hall that works as part of an integrated health and social care pathway for adults with learning disabilities that also have behaviours that challenge services. We are also working to support the Essex, Southend and Thurrock Transforming Care Partnership Board and as such the work of the team may involve w orking with people who are Ordinary Resident of Southend and Thurrock.

The current function and focus of the team is to work within the Transforming Care agenda to support adults who have been living in hospital services, particularly those identified following 'Winterbourne View'. This involves supporting adults who can move on to live in the community, and for those who are currently not ready to leave hospital, to ensure that they have a clear and progressive care pathway to potential discharge. This work is in line with ECC Adult Operations Charter vision of 'Helping People to help themselves, making personalisation real' - and our aim is to enable people to become more independent and less reliant on care services, and have 'Good lives'

The work involves holding a case load where you will have the opportunity to work closely with adults and their families to understand the issues they face, and then working to deliver creatively tailored solutions. This will include completing Care Act assessments and reviews, Carers Assessments, undertake Mental Capacity Assessments, and work in accordance with Deprivation of Liberty and Safeguards. You will also have the opportunity to work in partnership with Commissioners to develop new and innovative accommodation and support service for those we work with.

In addition the Team are responsible for completing Social Circumstance reports for Mental Health Tribunals and Hospital Managers Hearings, and to work in partnership with health colleagues in participating in care and Treatment Reviews and Care Programme Approach meetings.

The job can involve travel both across county and out of county and staff are encouraged to work flexibly. Successful applicants will receive regular individual and peer supervision and on-going consideration of development needs and training opportunities.

Knowledge, skills and experience
  • Diploma / Degree in Social Work
  • HCPC registered
  • At least 2 years experience
  • Experience of working with LD/Complex challenging behaviour


The role requires you to be mobile throughout a wide operational area including travelling around the county of Essex and beyond, transporting service users and their belongings and to be able to do so in an emergency. Driving licence and car or ability to meet the mobility requirements of the role through other means is essential.
