Interview Date: Week commencing 31 October 2016
Assessment, care planning, case recording, monitoring and reviewing of care plans for a case load of service users with disabilities/mental health conditions. Preparing reports, attending meetings as required for the benefit of service users within this case load. These tasks will be carried out in partnership with service users, family members, social work colleagues and other professionals, agencies as necessary to meet need.
Identify, assess and support suitable carers who will provide day support, respite and long term care for adults with disabilities/mental health conditions.
Fife Council pays the Scottish Local Government Living Wage Rate to employees earning less than £8.33 per hour. If the hourly rate of pay for the post is less than the Scottish Local Government Living Wage Rate a supplement will be paid.
Prepare assessments for adults with complex needs and develop creative care packages designed to meet these needs.
Carry a caseload with varying complexity requiring specialist skills and knowledge to enable the team to respond effectively to service users, with guidance from the Team Manager.
Conduct joint interviews with other agencies and other colleagues within the social work service. Provide reports in appropriate cases. Attendance at case conferences/reviews.
Before confirming appointment, you will require to become a member of the relevant PVG Scheme in addition to which registration with the Scottish Social Services Council is essential.

Workers new to role must gain registration with the Scottish Social Services Council (SSSC) within six months of start date in this role.
May be required to be available for appropriate duties such as reviewing cases, attending meetings or training sessions outside normal working hours.
Any other duties as required by the Executive Director Of Social Work and allocated by the Team Manager.
You will have a Diploma in Social Work or equivalent.
Knowledge and understanding of Adult Protection legislation, guidelines and information sharing protocols.
Experience of joint working to achieve common goals.
