Job Description:
Would you like to work with a primary health care team committed to delivering a more community-based care to frail older people and people living with long-term conditions so that hospital care is no longer the default option?
Our G.P partnership based in Bolton consists of 2 practices (only a mile apart) and prides itself on being a friendly place to work.
We are looking for an ANP or an NP that is an independent NMP. To work autonomously within their scope of competence to deliver excellent care to our patients with minor ailments and chronic conditions.
Would you like to manage your own caseload to assess, diagnose and treat patients autonomously while working within the context of a multi-disciplinary team committed to quality, evidence-based health care to achieve the best patient outcomes?
Desired Skills & Experience:
Job requirements
NMC Registration
Experience in Primary Care
Skills including: Prescribing, Minor Illness, chronic conditions.
Recognised Nurse Prescribing Qualification
To be accountable for their own practice including the assessment, diagnosis, treatment and discharge or referral of patients, independent of medical intervention.
To have a working knowledge of the modes of service delivery in primary care.
