CHC Nurse Assessor

Role: CHC Nurse Assessor/ Case Manager

Location: Lincolnshire

A great NHS Trust in Lincolnshire is looking for an experienced RGN/RMN/RGLN to join their growing CHC team.

Main Duties:

• Support the Locality Lead in the development of a range of services required for individuals who qualify for packages of care
• Case Managing CHC individuals
• Communicate information, risks, issues and dependencies, including briefings and reports.
• Procide professional nursing advice to acute hospital trusts, care providers and social sercices with regards to Continuing Healthcare.

Minumum Qualifications:

• Registered Nurse RGN, RMN or RNLD or Allied Health Professional with qualification
• Knowledge of NHS and social services guidance
• Strong knowledge base around safeguarding children and Adults
• Evidence of teaching/mentorship
• Clinical knowledge base in order to ensure comprehensive holistic assessment for continuing care
• Must have a UK driving Licence as travel county-wide is required at times.
